1. What is the eligible population for NHS Health Checks in the region under each PCT in the SHA?
2. Can you please provide the number of NHS Health Checks offered and completed by each PCT under the SHA in the FY 2010-11?
3. What percentage of NHS Health Checks were offered by GP practice, Community Providers and Pharmacies (if available)?
4. What are the targets allocated to each PCT for 2011-12?
