We are looking to send information to End of Life Care Facilitators within your SHA area, and I am unable to locate a directory of current contacts on your website.
Would it be possible to request a list contact details, as far as possible, at your earliest convenience please.

Under the Freedom of Information Act 2000 (General Rights of Access) that came into force on 1 January 2005, we have a duty to:
