Under FOI legislation please can you send me a list of email contact addresses for the chair of each of the GP consortia pathfinders that operate in your area. I do not require a personal email address but an email appropriate for me to send information relating to suggestions on future issues they should discuss at future executive meetings.

We have also recently updated our website to include the details of our five county-wide clusters made up of the nine Primary Care Trusts (PCTs). A map of the PCT clusters with information on their structures, including their Chief Executives and Chairs, can be found at the following link:
