I would like to make a request for information regarding the expression of interest to the Pathfinder Consortia Programme (related to the NHS White Paper "Equity and Excellence"), received by this Strategic Health Authority (NHS East Midlands).
Please could you provide the following:

A list of which Practice-based Commissioning Clusters/GP Commissioning Groups have submitted an expression of interest in joining the Pathfinder programme.

If any of the GP Commissioning Groups were created by merging more than one PBC Cluster, could you please indicate where this is the case.
And also could you please provide:

I refer to your email requesting information in respect of the Pathfinder Consortia Programme.
I can confirm in accordance with S.1 (1) of the Freedom of Information Act 2000 (FOIA) that we do not hold all of the information that you have requested. However, I will address each of the points that you raise in turn:
