Extenuating Circumstances Forms

Request

I would like to maker a request under the Freedom of Information Act.

The Strategic Health Authority had a deadline of August 12 to get the extenuating circumstances forms submitted by its trusts to the Care Quality Commission.

This is when a trust asks for an extension to or to be excused from reporting part of a datset required by the regulator.

While trusts make the request the SHA collates and sends them to the CQC. I would like to see copies of each form submitted by the SHA this year.

Our Response

Further to your request for information relating to the Extenuating Circumstances Forms submitted to the Strategic Health Authority (SHA) by Trusts within the area covered by NHS East Midlands, I can confirm that we hold the information you asked for and copies of all the forms received by NHS East Midlands for onward transmission to the CQC are attached.
