Please could I
have, under the terms of the FoI Act, a breakdown by
Trust of the
nutrition-related adverse incidents recorded on the
Reporting and
Learning system in each of the last 12 months.

Our Response
I refer to your
email of 15 January requesting information in respect of nutrition
related adverse incidents.
I can confirm in
accordance with S.1 (1) of the Freedom of Information Act 2000 that
we do not hold the information that you have requested. This
is because NHS East Midlands does not have access to the Reporting
and Learning System (RLS) system that you refer to.
However, the
information that you have requested is held individually by each
provider organisation and fed into the national RLS held by the
National Patient Safety Agency (NPSA).

I would also like
to take this opportunity to advise you that we have conducted a
thorough search on our Strategic Executive Information System

nutrition. We have found none in the time period
specified.
